Toledo Bend Family Medicine is a federally designated Rural Health Clinic devoted to keeping the whole family healthy.  

We offer a wide range of inpatient and outpatient services focused on prevention, early diagnosis, treatment and rehabilitation of illnesses and injuries. We are also committed to helping you better manage chronic health conditions and enjoy a healthier life.

Management and Treatment of Chronic Medical Conditions

Effective management and treatment of various chronic medical conditions is important to maintaining a healthy, active lifestyle, and the prevention and management of complications.  Some of the chronic medical conditions managed by your healthcare team at Toledo Bend Family Medicine.

Toledo Bend Family Medicine offers a wide range of diagnostic tests to assist with the development of your wellness or treatment plan. Services may be provided in the clinic or Sabine County Hospital .Our medical staff is here to serve your medical needs, Monday – Friday 8:00 a.m. to Noon and 1:00 p.m. to 5:00 p.m.  Each provider has a different schedule so for your convenience please call for an appointment.

Medicare Wellness Visits

If you are covered by Medicare, you are eligible for a free Annual Medicare Wellness Visit to talk with your provider about ways to improve your health and prevent disease.  The Annual Wellness Visit is not the same as an annual physical exam, but it does provide patients with the same opportunity to talk with their provider about health concerns and to develop a plan in partnership with those providers to focus on health, rather than illness. 

Some of the services covered during an Annual Wellness Visit include:  routine measurements such as weight, height and blood pressure; review of your individual and family medical history, review of your current medications, discussion of the care you receive from other providers, and review of your functional ability and level of safety.  Based on this review, you and your provider will develop a health promotion and disease prevention plan based on your specific needs.

After your initial wellness visit, you are eligible for a follow-up or subsequent wellness visit every 12 months.  These visits are not subject to copayments to deductibles, so there is no cost to the individual.  Call Toledo Bend Family Medicine at 409-787-1416 to schedule your Annual Wellness Visit.

AUTHORIZATION TO DISCLOSE HEALTH INFORMATION

Sabine County Hospital and Toledo Bend Family Medicine serve all patients regardless of ability to pay. 

Discounts for uninsured patients seeking medically necessary care provided by the hospital and clinic are offered based on the size of the patient’s immediate family, annual household income, and a sliding scale comparing household income with the hospital and clinic’s charity care income guidelines.  

Uninsured patients may apply for a discount in the hospital business office, or clinic front desk.

Our doctors contract with most major health insurance companies and participate in the plans these companies offer. A partial list of insurance companies with which we have contracts can be found below. Please feel free to call us if you have any questions.

If you have medical insurance, please provide that information to our staff when you make your appointment. We will verify your eligibility and file a claim on your behalf. Providing your insurance information in advance will help speed you through check in at the time of your appointment.

Copays, coinsurance and deductibles are due at the time of service. Not all services, even those most people consider routine, will be covered by every insurance plan. If you have questions about specific benefits, please contact your insurance carrier. Services not covered by your insurance plan are the patient’s responsibility.

Parents with children and adolescents enrolled in Medicaid and CHIPS can help to keep their family healthy through active participation in the Texas Health Steps program. Texas Health Steps or “T-Steps” offers routine checkups for children from birth to age 20, and pays for all costs.

The frequency and components of recommended T-Steps checkups vary by age, with more frequent visits encouraged in the first year of life. Beginning at age three, children and adolescents should be scheduled for an annual checkup. Although exams vary based on a child’s specific age, they include all medical screenings, vaccinations and laboratory tests recommended by the American Academy of Pediatrics.

Medical providers at Toledo Bend Family Medicine typically assess vision, hearing, height and weight, nutrition, immunizations and overall development, factors that are reflective of your child’s health status, and may impact your child’s performance in school, and during casual and organized play and sports. Toledo Bend Family Medicine is your local Texas Health Steps provider beginning with children age 6 months or older and can also meet the medical needs of children enrolled in the CHIP program.

The Chronic Care Management Program at Toledo Bend Family Medicine was established to help Medicare patients with diabetes, arthritis, high blood pressure, heart disease, stroke and other chronic conditions to play a more active and effective role in the management of their health.

Michelle Butler, LVN, a certified Clinic Health Coach, meets with each patient enrolled in the program to talk about their health care needs, concerns and goals.  Together, Michelle and the patient will develop an individualized plan to accomplish these goals, and will monitor progress over time, making adjustments to the plan as needed.

If you have a chronic condition, and would like to join the health improvement team, call 409-787-1418 to schedule an Annual Wellness Visit with your primary care provider to discuss enrollment. Michelle can also be reached at the same number to answer any other questions you may have about the program.

Coaching services provided by Michelle are offered free-of-charge to eligible Medicare patients.

Greg Backofen

Greg Backofen, M.D.

Greg Backofen, M.D., has been a member of the provider team at Toledo Bend Family Medicine since 2015. He completed his family practice residency at the University of Arkansas for Medical Sciences (UAMS). The “rural track” offered by UAMS allowed him to rotate through several smaller communities including Magnolia, Arkansas. During his three-year tenure at UAMS, he was selected as chief resident for two consecutive years. Fluent in Spanish, Dr. Backofen attended medical school and completed an undergraduate degree in pre-medicine at the Universidad Iberoamericana Medical School, Santo Domingo, Dominican Republic. He also has a bachelor’s degree in finance from Louisiana State University. He and his wife Brandy, a former pharmaceutical sales representative, live in Hemphill along with their two golden doodles who also enjoy the advantages of Hemphill lake and recreational areas.

Chris Melcher

Chris Melcher, PA-C

Chris Melcher, PA-C, has been a member of the provider staff at Sabine County Hospital since 2011, covering the emergency department and providing care to clinic patients. He relocated to Toledo Bend Family Medicine in 2017. Chris entered physician assistant school after spending 21 years in the U.S. Air Force where he served as a Medical Services Technician responsible for supporting USAF personnel in remote locations worldwide. In addition to his military experience, Chris also worked as a physician assistant at Sparta Community Hospital in Sparta, Illinois, and the Carterville Family Practice also in Carterville. Chris graduated from the Southern Illinois University Physicians Assistant Program with a bachelor's in science, received his second associate's degree in arts and science from Pierce College and his first associate's degree in applied science, allied health sciences from the Community College of the Air Force. Chris and his wife, Diane, have one grown son living in Illinois.

Chad Moody

Chad Moody, MS, PA-C

Chad Moody, MS, PA-C, was born and raised in Sabine County, and joined the staff of Sabine County Hospital and clinics in 2009 to serve the residents of his hometown. He provides family care to patients at Toledo Bend Family Medicine, and also covers the emergency department of the hospital. Prior to joining Sabine County Hospital, Chad worked in the emergency rooms at Memorial Health System of East Texas in Livingston and Lufkin, and at Watson Family Medicine in Hemphill. Chad received his Masters of Science degree from Baylor College of Medicine in Houston in December 2005 and successfully completed his Physician Assistant Program certification with the same school. He also maintains advanced certification in emergency medicine. Chad and his wife, Jennifer, are the proud parents of three children.

Mareike Gay, FNP-C

Mareike Gay, FNP-C, joined the team of medical providers at Toledo Bend Family Medicine in November 2020 after earning her Master of Science in Nursing – Family Nurse Practitioner degree at Walden University. Gay worked at Sabine County Hospital in various nursing positions since 2004, and was eventually promoted to Chief Nursing Officer. She stepped down from her position as CNO in 2018 to complete her nurse practitioner studies. While at school, she continued to work as a floor nurse and on special projects at SCH. A native of Germany, Gay graduated from nursing school in 1999 and worked at a German hospital as a surgical nurse for the next three years. Married to Chad Gay and mother to Amelia, age 12 and Myah, age 11, she is also raising a family in Hemphill.

Valerie Upshaw, APRN-CNP

Valerie Upshaw, APRN-CNP is an experienced nurse with over 10 years of clinical research and experience. While in training for her Nurse Practitioner License, Valerie worked in Tulsa, Oklahoma as a researcher for the Laureate Institute for Brain Research. Valerie moved to the Hemphill area because her husband Andrew is from the Sabine County area and has a lot of family around Hemphill. Valerie particularly enjoys currently seeing patients for women’s health issues, hypertension, as well as managing diabetes.
